You would be the first to define what a high-standard orthopedic service looks like in this context — from the implants on the shelf to the protocols in the theatre. You'd manage complex fracture, joint, and spinal cases while simultaneously designing the service that will handle them at scale.

Neurological disease is consistently underdiagnosed and undertreated in the region. Advanced neuro diagnostics — MRI, CSF analysis, electrophysiology — are largely inaccessible. You'd define the neurology service from the ground up: the diagnostic pathway, the case management approach, and the criteria for what gets referred versus managed in-house.

Anaesthesia sits at the intersection of every surgical and procedural service. Without a specialist-designed anaesthesia programme, advanced surgery is impossible. You'd establish the anaesthetic standards that underpin the entire clinical operation — from TIVA protocols to locoregional techniques to high-risk patient management — and advise on the monitoring and equipment stack to match.

You'd build the cardiology service from scratch — defining the diagnostic pathway, the formulary, and the standards by which cardiac patients are monitored and managed long-term.

You'd design all of it: the physical space, the clinical workflows, the drug and equipment standards, and the escalation criteria that connect ECC to every other specialty.
