You would be the first to define what a high-standard orthopedic service looks like in this context — from the implants on the shelf to the protocols in the theatre. You'd manage complex fracture, joint, and spinal cases while simultaneously designing the service that will handle them at scale.

Neurological disease is consistently underdiagnosed and undertreated in the region. Advanced neuro diagnostics — MRI, CSF analysis, electrophysiology — are largely inaccessible. You'd define the neurology service from the ground up: the diagnostic pathway, the case management approach, and the criteria for what gets referred versus managed in-house.

Anaesthesia sits at the intersection of every surgical and procedural service. Without a specialist-designed anaesthesia programme, advanced surgery is impossible. You'd establish the anaesthetic standards that underpin the entire clinical operation — from TIVA protocols to locoregional techniques to high-risk patient management — and advise on the monitoring and equipment stack to match.

Cardiac disease in small animals is common and consistently managed without specialist input in this region. Echocardiography, Holter monitoring, and evidence-based medical management are effectively unavailable. You'd build the cardiology service from scratch — defining the diagnostic pathway, the formulary, and the standards by which cardiac patients are monitored and managed long-term.

Advanced soft tissue surgery — oncological resection, hepatic and splenic surgery, intestinal reconstruction, complex wound management — requires both a skilled surgeon and a designed service around them. You'd define the surgical service: what cases get taken on, what the pre- and post-operative care looks like, and how the theatre is set up to support it.

Internal medicine is the backbone of any advanced referral service. Complex, multi-system, and chronic disease patients need structured diagnostic thinking, access to advanced diagnostics, and consistent long-term management. You'd define the internal medicine service: the diagnostic workup standards, the approach to endoscopy, ultrasound, and biopsy, and the protocols that keep complex patients well-managed over time.

ECC is the entry point for the sickest patients — and the department that sets the tone for clinical capability across the entire hospital. A well-designed emergency service requires triage systems, ICU architecture, monitoring protocols, and a team culture built around speed and precision. You'd design all of it: the physical space, the clinical workflows, the drug and equipment standards, and the escalation criteria that connect ECC to every other specialty.
